Forms Required for Every Participant:
Several forms are required for every participant in a Mountain Workshop summer camp. After registration is completed, all paperwork required for your program will be sent to you. We ask that all health forms and waivers be returned by May 1, 2012 or as soon as possible.
- A parent or guardian is required to sign a Parental Permission and Release form.
- Various waivers are required by certain outfitters or locations we use.
- As required by the State of Connecticut, a Health Exam form, which includes immunization records, must be completed by a physician. This form may be up to three years old. You may substitute The Mountain Workshop's Health Exam form with another form that includes the same information, such as the CT school blue health form.
- Parents are also asked to complete a Health Information form to inform our staff of any allergies, conditions and medications.
- If a camper will be taking ANY medication (prescription or over-the-counter) while on a Mountain Workshop program, an Authorization for Administration of Medication form must be completed and signed by the camper's physician as well as a parent or guardian.
Terms and Conditions
Refund Policy
If you choose to cancel your program prior to May 1, 2012, you will be refunded the amount paid, minus $100 administration fee. The Mountain Workshop will issue no refunds after May 1, 2012 for any reason. Pro-rated deductions or rebates for early withdrawal, late arrival, or dismissal are not available. No refund will be granted should a camper withdraw voluntarily, for whatever reason, after the start of a trip. Please recognize that our cancellation and refund policies are based on unrecoverable expenses of outfitter fees, food and equipment pre-purchases, staff training, site or equipment reservations, and other expenses incurred prior to a camper's attendance. Additionally, late season cancellations typically do not allow time for another camper to fill the vacancy.
Trip Cancellation Insurance Recommended
Campers are encouraged to purchase a travel insurance plan, such as the Travmark Travel Protection Plan or other plan of your choice. Travel insurance benefits are designed to protect you for situations or losses resulting from sudden and unexpected conditions or events.
Optional program cancellation insurance for The Mountain Workshop participants is offered through TravMark. For information about purchasing this policy go to www.travmark.com. Organization name: Mountain Workshop, Organization ID#: mntn11
Cancellation and Alteration of Programs
If a day camp or overnight expedition trip is canceled for any reason prior to its start, The Mountain Workshop will refund all payments. If it becomes necessary to cancel a program once it has begun, the portion of the unused payment will be refunded. Our intention is to follow the itineraries as described in our brochure. The order of activities may be changed due to scheduling conflicts, weather or equipment considerations.
Rules and Regulations
Campers will be expected to commit to a verbal contract at the beginning of the program. This contract will include a commitment to guidelines of behavior for the safety and well being of the individual and group. These include the prohibition of all forms of tobacco, alcohol and illegal drugs, cooperation with group leaders and other members of the group, and the commitment to not become involved in cliques and mutually exclusive one-on-one relationships and excessive displays of affection. The trip leaders will handle discipline problems in the manner that they see fit, according to industry guidelines. If a camper is dismissed for the well being of the program or for failure to cooperate, parents will be called and are responsible to pick up their child immediately and at their own expense. No refund will be given.
Equipment
The parent/guardian is responsible for and agrees to reimbursement for loss of or willful destruction by the participant of any equipment belonging to The Mountain Workshop. The Mountain Workshop will not be responsible for any participant's possessions that are lost or stolen while he/she is on the trip.
